shipping with DHL?

I have a prepaid shipping page from a bank that I am supposed to send some financial info to. I am not sure how to use it. I need to drop it off by tomorrow and am not going to be able to call by then. It is just a sheet of paper with the shipping info on it along with a barcode and codes and stuff. Do I just tape or staple this to the envelope to mail it? Any help would be appreciated!

1. I am a little confused, but I would suggest using a staple because it is too easy for tape to come undone.
2. They have DHL envelopes at the gold DHL box drops. Make sure there is a pick up at the location -- they close those boxes for use all the time! Bring tape and a spare envelope with you in case they are out of them; in that case, just tape the sheet to an envelope big enough to hold it. And use the right drawer/slot for the envelope -- I almost dropped it off in the wrong place the first time I used that stupid box.
