what is a UPS shipping waybill ?

Im sending my ipod to apple for repairs and in the instructions it says "complete the enclosed UPS shipping waybill..." and i have no idea what it is? some one please help me since i have to send this package out today! i understand what it is now, but it says "enclosed" but i cant find anyhitng that has anything fill out in the box... weird? would it be possible to get this at the fedex where im shipping it off from?

1. When you ship through UPS, or even Fedex, you have to fill in a document with all relevant information including name, address, contact no. of the sender and receiver, declaration of what is being sent and its value... This is the waybill.
2. What the above answers have not mentioned is that you should receive a tracking no. that can be used to confirm delivery for both parties, and will allow you to see when it arrives, and to contact them to get an estimate of the return date.
